The Orange Park Police Department sincerely values your opinion and loves to hear from our community. We encourage you to tell us of positive encounters you have had with our members, provide us with information that may prove helpful in preventing/solving crime or make us aware of an issue that needs to be addressed.

Complaint
Our Investigations Division is tasked with the formal Internal Affairs function of our department and Supervisors are tasked with internal reviews that do not reach the level of formal internal investigation. If you have a complaint regarding one of our employees, you may ask to speak with any on-duty OPPD supervisor directly at any time by calling 904-264-5555. Citizens may also obtain and complete a formal complaint form which is attached below. After you print and complete the form, return it to the Orange Park Police Department where it can be followed up on.
